Position Title:  Community Health Worker

Position Available:  Full-time (40 hours/ week) position in Washington, DC. Employment is contingent upon funding.

Program Overview:  IPHI is seeking to hire a Community Health Worker to support a contract with MedStar Washington Hospital Center’s (MWHC) Ryan White Part A Program. Per an agreement with the DC Department of Health, MWHC will create partnerships with community-based public health organizations to provide high impact prevention, early intervention for at-risk individuals, navigation support, and linkage to care.

Reports to: IPHI Program Manager and MWHC Clinical Social Worker

Job Summary: The Community Health Worker will use their unique position as a trained community member to provide trust-based information and education, walk clients through the often-complex healthcare system, and help them strategize to manage the logistics of caring for oneself in the context of complicated health issues. This position will also include a significant educational opportunity for the newly hired CHW. We expect that the hired community health worker will benefit from a core 100-hour CHW training within the first few months of employment to prepare for their positions, as well as ongoing continuing education and training to facilitate success in their work.

Primary Roles and Responsibilities:

  • Facilitate and coordinate care, home visits and referrals with clients together with MWHC social workers or medical case managers.
  • Provide linkage and referrals for support services for those falling out of care and requiring reengagement strategies
  • Provide non-medical case management support services and guide high-risk individuals to connect with local community organizations.
  • Develop an understanding of and relationships with local community organizations that offer services such as health education, testing of exposed partners meeting locations, health insurance navigation, and PrEP information clinics.
  • Identify resources for those recently diagnosed and falling out of care in need of behavioral and substance abuse services, need for housing, food, clothing, or emergency financial assistance.
  • Maintains and follows proper protocol for documenting member encounters, activity logs, and other back-office responsibilities in an accurate and timely manner.
  • Maintain patient charts and document encounters in MedConnect and CAREWare per MedStar’s schedule.
  • Participate in MedStar team meetings.
  • Perform other duties as assigned.

Qualifications:

  • Experience and Interests:
    • Demonstrated experience with community outreach.
    • Previous position or volunteer work in HIV services preferred.
    • Experience with issues that put people at risk for poorer health such as homelessness, incarceration, domestic violence, and/or substance abuse is preferred.
  • Skills:
    • Strong verbal and written communication skills including excellent listening skills. Strong peer counseling and relationship building skills, and a demonstrated ability to conduct sensitive, empathetic interviews and conversations that highly respect the dignity and diversity of clients. Ability to engage staff and clients in a supportive and empowering way. Ability to help manage conflict. Ability to maintain client records and information in an accurate, timely and confidential manner. Scheduling, planning, and follow-up skills. Ability to interface effectively with a diverse population. Ability to identify and access resources. Strong organizational and time management skills. Basic computer skills required. Ability to work well with people from a variety of professional backgrounds.
  • Personal qualities:
    • Personal qualities including energy, intelligence, self-motivated, detail-oriented, diplomacy, sound practical judgment, the ability to work independently during periods of less supervision; the ability to work effectively in a busy environment; and the interpersonal skills to work effectively in partnership with other program staff and others in the agency. Ability to work independently and with teams. Resourcefulness will be required to assist individuals, as well as good judgment and decision making.

 

Education Required: 

High school diploma or GED required.

Salary and Benefits:

Salary is contingent upon available funding and is commensurate with experience and qualifications. The starting salary for this full-time position is expected to fall in the range of $36,000 – $39,500. Job Location:

The community health worker will be employed by the Institute for Public Health Innovation (www.institutephi.org). The position requires the employee to work at MedStar Washington Hospital Center in the District of Columbia. The Community Health Worker will be required to travel to throughout suburban Maryland and Virginia to conduct home visits and connect to community-based organizations offering services that support MWHC Ryan White Part A clients, and thus a valid driver’s license and access to a vehicle are highly preferred.
