Heart to Hand, Inc.
Heart to Hand, Inc. is a grassroots non-profit organization located in Prince George’s County, Maryland that focuses on empowering communities of color with model health care systems that promote and support health and wellness. Heart to Hand was established in 1998 by two concerned African-American women who saw the need to provide a voice to the voiceless. HIV/AIDS was spreading throughout the community; specifically African-American women were becoming HIV-positive at an alarming rate (and still are). In 2013, IPHI and Heart to Hand established a partnership to enhance outreach to community members in order to ensure that all HIV-positive individuals were aware of their status and were effectively engaged in medical care and other support services. Through several initiatives, IPHI employs community health workers who work from a home base at Heart to Hand’s office with support from Heart to Hand staff. IPHI and Heart to Hand collaborate with other key health care partners, including Greater Baden Medical Services, Dimensions Health System, and the Prince George’s County Health Department. These initiatives are funded by AIDS United, Kaiser Permanente National Community Benefit Fund at the East Bay Community Foundation , MAC AIDS Fund, and Washington AIDS Partnership.
