IPHI to Serve as Primary Partner on $12 Million CDC Cooperative Agreement to Improve Chronic Disease in Southern Maryland
PreventionLink is a new regional initiative in Southern Maryland funded by a five-year cooperative agreement from the Centers for Disease Control and Prevention (CDC) with the goal of strengthening clinical and public health interventions to help over a million residents prevent or manage diabetes, heart disease, and stroke. The project is led by the Prince George’s County Health Department in collaboration with a broad range of public and private sector partners across Prince George’s, Calvert, Charles, and St. Mary’s counties. IPHI is a primary PreventionLink partner and will provide technical assistance, training, and project management support.
