IPHI Works Closely with Health Care Providers Throughout the Region
IPHI works closely with health care providers throughout the region to integrate community-based public health strategies into medical care service systems. In particular, we are involved in training and supporting community health workers and the clinical staff with whom they work in order to implement more effective models of community outreach, patient education and engagement, and disease management. We also provide technical support and partnership opportunities to medical providers that are interested in implementing patient-centered medical home models and addressing social determinants of health in the communities within which they work.
IPHI is proud to have partnered with dozens of health care providers across the region, including but not limited to: Alexandria Neighborhood Health Services, Bon Secours Baltimore Health System, Community Clinic, Family & Medical Counseling Services, The George Washington University Hospital, Greater Baden Medical Services, Inova Health System, MedStar St. Mary’s Hospital, MedStar Washington Hospital Center, Unity Health Care, and Whitman-Walker Health.
